This questionnaire has been developed to rule out major contraindications and to get a complete medical and psychiatric history. There is no charge to submit this private and protected form for review. If the results from this questionnaire show that you are a good candidate for ketamine, you will be scheduled to meet with the psychiatrist to go over your history, create an individualized treatment plan, and thoroughly discuss ketamine treatment with you.

This initial meeting includes an evaluation, screening, and first infusion for $500.
